TheBigWild.org has put up posters through Canada that include QR Codes as the call-to-action. (From their website: “The Big Wild is a conservation movement that makes it easy for you to stand up for wilderness protection campaigns.”)

Some of the codes point to mobile-friendly landing pages (like this one) that enable people to easily fill out the petition form.
